NK moves in to SK territory with a ground war in order to gain a unified Korea....
Kim tells the world "Do not interfere! We WILL launch nukes at any country that does."
Sanctions will speed this up - NK doesn't want its troops starving more, so they want that ground war as soon as possible.
A diesel engine submarine running on battery power runs silently - there could be a few already with the nukes that have been tested around American coasts right now.
He probably took the chance that they would work, and set them on their travels weeks ago. If the tests failed he'd order the subs back.
NK than takes control of SK's resources to negate the sanctions that had been in place.

America can't strike first, China's told them it wouldn't support them if they did.
America absolutely refuses to have NK increase it's influence via nukes, and gaining territory.
The only solution I see is a False-Flag (America making NK look like it fired on Japan or South Korea), so it and China will be able to surgically strike all of NK's nuke facilities.
Of course - intelligence might have discovered these nukes already mounted on missiles, and the tests were retro-active!
In this case - no False-Flag, as NK would likely launch nukes as it has all to lose from the surgical strike.
