Overview
- Date of accident: around 6:25 on August 8, 2015
- Worker: a male 52 years old
-Company: a contracted company of Kajima Corporation (contractor)

Situation
A contracted worker who was cleaning the vehicles used to construct the landside impermeable wall in the soil disposal area at Fukushima Daiichi Nuclear Power Station has injured, being caught his head in the tank hatch placed on the vehicle's rear part.

Timeline

06:15 - Start of operation
...
around 06:25 - The accident occurred
06:27 - A call to the emergency medical room (ER) on the premises of Fukushima Daiichi Nuclear Power Station
06:36 - A request for an ambulance from ER to Futaba fire department headquarters
06:42 - A doctor arrived at the site from ER
07:00 - Start of carrying him to ER via a rescue vehicle
07:03 - Arrival of an ambulance to ER in the Entrance Control Building
07:05 - Arrival of a rescue vehicle to the ER
07:27 - Departure of the ambulance from Fukushima Daiichi
07:56 - Confirmed his death at the conveyance destination hospital

The timeline looks screwy to me. It seemed to take a long time for them to get the ambulance there, and to get to the hospital.

Looking at pictures of the vehicles he was cleaning, I can't see what "hatch" they are talking about him getting his head stuck in, or how that could have killed him.

Unfortunately, I doubt we'll be told much more than we have already. Could the radiation be taking a toll on the workers cognition, making them more accident prone?

I doubt we'll ever know the true number of workers who've died at Fukushima. The use of contractors makes it fairly difficult to find out anything about the employee's or their health.

What i don't understand:

This Construction Site was in the very high Contaminated Ring of the Ice-Wall, a few Meter from the Reactor Buildings, this Area is fully covered
with Cameras, why do they not saw it coming?

Point Two:
Japanese Construction Cars make a hell of Noise when they go Backwards
or/ and operate their Working Tools (normally a high pitched Warning Sound)
Does it work in that Time and if so why this Worker do not observed this Warning?

Point Three:
Why they was allowed to work with two People only?
When they work here they need to have at least two Security People
who control the Area!
