I don't think it's Val Kilmer. He's packed on a lot of weight and has some type of throat tumor/cancer thing going on. I've also never heard any rumors about him. RDJ & Sheen fit the details most closely, although neither one would be a huge surprise. It sucks that celebrities have to worry about a serious medical issue ruining their career. :(
