Easily overcome. Have your doc write it for 2x the mg, then cut the tablets in half and take half a pill 2x daily -- all of which will be covered by insurance.

One you will be forced to pay for yourself the other the insurance company will pay.

What you really need to do is have the Doctor appeal the decision by insurance to limit it to one dose a day up to two doses a day.
They will bitch and complain whole way.
Keep appealing they will eventually cave if your doctor send the needed proof for the increase.
