There would have been a minimum of two sailors outside of the bridge, one fore and one aft, as backup for the electronic systems had those been compromised. No excuse for visually missing a 600 ft tanker...even if radar/gps other systems not working properly. Someone (or more than one) simply messed up. Either through poor training or inatention.
