Not necessarily. There are many factors. If it is a limited strike, air burst, h-bomb, there's a good chance you can live a full life after the attack, if you survive the blast.

Don't give up hope if you survive a thermo-nuclear blast.

It it's a small blast, and you are close enough to feel the blast, you're more likely facing an enhanced radiation weapon (possibly a neutron bomb). That is a worse prospect, but you still have a chance if you are sheltered. Just don't have any babies after.
