Those who can't afford healthcare insurance (on their own or through their employer) can get the equivalent of your healthcare which we call Medicaid in the US. It's the downgraded version of our healthcare system usually with long waits for procedures and usually they'll not get any of the cutting edge treatments do to cost (like your NHS).
