The problem with his rocket is that it wasn't wide enough for him to be seated supine inside of it. The initial G forces of launch killed him. Because his legs were above him, upon acceleration the blood in them would rush to his head, causing him to black out. I bet the parachute was set up as part of a dead man switch, if he blacked out, the parachute deployed. I suspect Evel Knievel, used a similar device for the Snake River jump. The problem with these sort of rockets is that they have almost instant acceleration instead of a gradual one. That Mad Mike survive a previous launch is remarkable.
