I don't know much about how metadata works, but if the metadata isn't showing anything being messed with or changed, how did the large arrows and circles get into the image? It has obviously been changed from the original just to add those.

Valid question, the original image was taken by a Motorola Edge.

I captured the image and downloaded it onto my laptop.

The image was printed, arrows added, and then I took a photo with my iPad.

The software used to determine inconsistencies in the metadata doesn’t include variances for anything added to the top layer beyond the picture or image itself.

Basically the software ignores the arrows and circles because they are not actually a part of the image itself.
