NHS must not stop normal care when a second wave of Covid-19 hits - or tens of thousands of patients may die, leading medics warn

In early March the NHS told hospitals to scrap as many operations as possible and turf out patients on their wards to make way for an influx of Covid-19 patients.

The move was successful and hospitals were not overwhelmed by the effects of the virus. But cancer charities fear tens of thousands more patients will die in the next year because they had their tumours diagnosed too late or missed completely.

NHS is accused of wasting 'eye-watering' amount of money as main 'test and trace' contract costs taxpayers £900 per person contacted

Private firm Serco has a £108million 13-week deal to get in touch with people who test positive and their close contacts and encourage them to isolate for two weeks.

But figures uncovered by Labour show that after nine weeks only 83,000 people have been contacted, at a cost to the taxpayer of £902.50 for each one.

The NHS test and trace system has been beset by delays and is failing to reach enough people to be effective.

Thousands of contact tracers have been employed but many have no work to do. The Serco revelation comes a day after it emerged the government had paid £150million for masks that cannot be used.

Two thirds of staff at one NHS hospital trust lost their sense of taste or smell during the peak of the coronavirus epidemic but most couldn't get tested, a study shows.

Researchers asked staff at London's Barts Health NHS Trust to complete a questionnaire in the week of April 17 to 23, at the height of the epidemic.

Almost two-thirds of staff at the hospital said they had suffered anosmia - a loss of taste or smell.

Apparantly the first test was antigen, second test PCR. So he's most likely negative because PCR have the higher accuracy. What I don't get is the PCR test was administered in the afternoon but they have the result back already? I thought it took 24 hours minimum to process a PCR test.

Exactly. If Dutchy is right in what he says that the more accurate PCR test only takes 1-2 hours to process then why aren't they using that in the first place.

The rapid test (the one with lower level of accuracy) is cheaper I think. But c'mon, if the Governor was due to meet with the President just give him the PCR test. It turns out the rapid test gave a false positive, but it could just as well have been a false negative. (I think I've got that right Dutchy?)

Antigen tests are point of care. You take the sample apply it to a test strip or put the sample in a machine and you have a result. Takes ~15 minutes to get a result. And doesn't require many additional resources or staff. They test for the presence of specific proteins associated with the virus. You can easily do it routinely for anyone that comes into contact with someone. And running a single sample is feasible. But there are indeed more false results both ways.

PCR testing takes a sample, breaks down the envelope of the virus and then synthesizes the viral RNA with nucleic acid amplification if it is present. It's more resource intensive and you need trained lab techs working in a decently controlled environment.
- Take sample->Prepare the sample via reagents->Add the primer that copies the sequence over X cycles->Thermal Cycle the sample with the primer to stimulate the cycling.
It takes a bigger machine and more time but it's biggest limitation is that you need to do it in a controlled environment and preferably run larger batches of samples at the same time, you can setup a lab but you cannot easily move the lab and the machines/resources used are generally in short supply.

Antigen test (Costs $1-$10) -> Quick and easy to perform (15m-1hr at point of care) anywhere but less accurate, useful for quick results where ever you are. If you can test people daily you can negate the effect of false results. It's a bad test to determine if a single person is infected if used irregularly though. Where they are very useful is in I.E. a school/college where you have bubbles and can use them to screen a group to assess if there are infections present daily. One positive->PCR test the entire bubble to determine who/how many.

PCR tests (Costs $100 to run the test in a lab, not adding the cost of sample collection (Nurses/Storage of Samples/Transport of samples) -> Slower (processing the sample and cycling it takes 1-2 hrs, but the sample needs to be taken and transported to a lab, prepared, processed afterwards) and very hard to scale (finite number of PCR machines that require lab technicians, scaling up takes a large initial investment) but if the virus is present in sufficient amounts to signify active infection you will find it. Great for determining the presence of the virus in a single person at the point in time the sample was taken.

So Antigen tests are fast, cheap ($ and resources) and scalable at the cost of accuracy.

PCR testing is slower, expensive ($ and resources) and hard to scale but more accurate.

The entire accuracy discussion could easily extend this post to the length of a essay so i'll forego detailing it, both tests have their own limitations and short comings but PCR is more accurate then Antigen.

As for the decision in the white house? I think the speed is a important factor if someone needs to see El Presidente having a quick result can be important, waiting for 15 minutes for a result that is processed down the hall is easier then taking a sample sending it to the lab etc.

Yea, if White House staffers had to meet with Trump during the same day then I'd think antigen would be suitable. But the Pres going on an arranged visit to meet with a Gov in another state where cases have been increasing recently, I would have thought a PCR taken the evening before would have been the best option. Last weekend in the Manchester area we had a case identified with a large employer. The outbreak control team implemented mass testing. In accordance with the plan, mobile testing support was requested. That don't arrive until yesterday.

People with symptomless Covid-19 can carry as much of the virus as those with symptoms, a South Korean study has suggested.

South Korea was able to identify and isolate asymptomatic cases through mass testing as early as the start of March.

There is mounting evidence these cases represent a considerable proportion of coronavirus infections.

But the researchers weren't able to say how much these people actually passed the virus on.

People with a positive coronavirus test were monitored in a community treatment centre, allowing scientists to look at how much of the virus was detectable in their nose and throat swabs.

They were given regular tests, and only released once they were negative.

Results of 1,886 tests suggest people with no symptoms at the time of the test, including those who never go on to develop symptoms, have the same amount of viral material in their nose and throat as people with symptoms.

The study also showed the virus could be detected in asymptomatic people for significant periods of time - although they appeared to clear it from their systems slightly faster than people with symptoms.

A second attempt at a Covid-19 contact-tracing app for England will soon be tested by members of the public.

Officials hope to confirm the date for the limited roll-out within a few days. It could be as soon as next week.

The app will let people scan barcode-like QR codes to log venue visits, as well as implementing Apple and Google's method of detecting other smartphones.

But efforts are still ongoing to deliver medical test results within the product.

So this means that for efficient contact tracing you will need 1.5 contact tracer days in followup (Assuming 8 hour work day). At the time you start getting overwhelmed it takes 3-4 weeks to scale up, in those same 3-4 weeks you can see cases double 3 or 4 times.

Combine that with the above data about taking multiple days to setup a testing site at a point of outbreak etc.
