Very true. Part of the doctor's reaction is subconsciously identifying and sympathizing with the lower, unhealthy class he saw. He did not want to be identified with them and yet they are turning into the majority and therefore must either be appreciated or disdained. He is saying he can see why the elites disdain them though part of him identifies with the underclass. Many educated Americans refuse to enter Walmart, Americans much closer to Walmart demographics than the doctor and who certainly identify more closely with those they are disdaining.
