The mark of the Beast happens after the 3.5 year mark of the Tribulation. The perfect world the antichrist envisioned is falling apart totally and he goes into the J---ish temple and declares himself god on earth and desecrated the temple. He then implements the mark of the Beast and anyone not agreeing with everything he says and does is killed. To show allegiance to the AC, a person has to take a mark showing they are part of his empire. A person will know if they take it, it will be in allegiance to him, not a medical thing
