Providers are not forced to take government insurance. Medicare and medicaid pay pennies on the dollar. Dropping a couple percent off the bottom line is not going to hurt them much. Its a silly dictate by Xiden and the only people who are going to suffer are Medicare and Medicaid patients.

I was considering looking around at independent facilities, dialysis, senior care etc.

But all those take Medicare/medicaid.

So unless I can find a self pay provider that doesn't participate in Medicare/Medicaid forget it.

Maybe a high end plastic surgeons office?

That is already happening on a smaller scale, for now. My best friend cannot get their painful legs treated because they refuse the jab. FTR, my friend is diabetic(not overweight) and had a previous heart attack.

Keep an eye out for the large health care entities that also sell life insurance particularly to their employees. They have been slow to move on mandating the vax because they will be hemorrhaging cash when their employees start dropping dead.
