The same goes for organ transplants. The donor needs to be kept in a working state while the organs are removed. They cannot give the donor any anaesthetic because it would spoil the organ to be removed. That's probably why they call the donor as "brain dead" but the poor bastard would be feeling every incision and cut during the organ removal. Ouch!
