A deadly cousin of Ebola, Marburg can kill nine out of ten people it infects, and international travel has taken it from Africa to Europe twice in the past 40 years. The virus has just hit Equatorial Guinea for the first time ever – what does this mean for the rest of the world?

The largest known outbreak of Marburg virus, in Angola in 2004, infected over 250 people and had a 90 percent fatality rate.

Pandemic threat:

As Marburg virus can spread from human to human through contact of bodily fluids, much like Ebola. As outbreaks in Europe and the US have already shown, increasing globalisation and international travel mean that the risk for global spread is high, especially when the incubation period could be up to three weeks. This could be disastrous given its high death rate.

How is it spread?

The Egyptian rousette fruit bats often harbour the virus. African green monkeys have in the past spread the virus to people in Uganda, but pigs can also become infected and can be a source of infection. Marburg virus is spread through direct contact (through broken skin or mucous membranes) with the blood, secretions, organs or other bodily fluids of infected people, and also through any materials such as bedding, that have been contaminated with the infected fluids. As a result, health workers have often become infected by treating patients with Marburg virus. Burial ceremonies in which people have direct contact with the body can also drive the spread of the virus.

Two more people in Equatorial Guinea have died of Marburg hemorrhagic fever, a cousin of the Ebola virus, bringing the toll of fatalities to 11, the authorities say.

"Two days ago, the monitoring system recorded eight notifications, including the deaths of two people with symptoms of the disease," Health Minister Mitoha Ondo'o Ayekaba said in a statement issued late Tuesday.

Work is underway "to strengthen assessment of the spread of the epidemic," said the statement, read on national television.

"Forty-eight contact cases have been documented, four of whom have developed symptoms, and three who have been quarantined in hospital," it added.

This is a particularly dangerous and highly contagious disease, with the mortality rate reaching up to 88%, the ministry said in a dispatch to localities nationwide.

Localities are requested to strengthen supervision over entrants from other countries, especially from African countries with Marburg virus disease outbreaks, for 21 days to swiftly detect suspected cases.

Furthermore, they also need to proactively develop scenarios in response to any outbreaks occurring locally, and at the same time prepare sufficient medical equipment, human resources, and funding to effectively cope with a possible epidemic.

Africa is grappling with not one, but two outbreaks of Marburg fever, a disease that causes symptoms and a death rate comparable to Ebola, its viral cousin.

Health officials in Tanzania announced Tuesday that they had confirmed the country’s first-ever Marburg outbreak, involving at least eight people so far, five of whom have died. One of the people who died is a health care worker.

Across the continent, Equatorial Guinea has been combating its first-ever Marburg outbreak for several weeks now, though in an unusual turn of events for a viral hemorrhagic fever outbreak, scant information has been shared with the international community. The last update from the World Health Organization on that outbreak was issued nearly a month ago, on Feb. 25.

Tanzanian Health Minister Ummy Mwalimu announced that the country’s national public health laboratory had confirmed that Marburg was the cause of an outbreak that was first reported last week. Five members of a single family are among the cases. At least 161 people who have been in contact with the cases have been identified and are being monitored.

Most of the ebola/marburg type viruses are not airborne though correct? But the main fear is that they will evolve to become airborne therefore more easily transmissible?

Since the first Disease Outbreak News on this event was published on 25 February 2023, eight additional laboratory-confirmed cases of Marburg virus disease (MVD) have been reported in Equatorial Guinea. This brings the total to nine laboratory-confirmed cases and 20 probable cases since the declaration of the outbreak in February 2023. There are seven deaths among the laboratory confirmed, and all probable cases are dead. Of the eight new confirmed cases, two were reported from the province of Kié-Ntem, four from the Litoral, and two from Centre- Sur provinces. The areas reporting cases are about 150 kilometers apart, suggesting wider transmission of the virus.

Kenya: The Ministry has thus cautioned about the impact of MVD, which is scientifically classified under Viral Hemorrhagic Fever (VHF) and is in the same category as the Ebola Virus, both in symptoms and transmission.

“Marburg Virus Disease (MVD) is often a serious and fatal disease caused by the Marburg virus and has a Case Fatality Ratio (CFR) of up to 88%, but can be much lower with good patient care,” he said.

Marburg has its roots in West Africa and its patients are said to have often between 8 and 9 days after symptom onset up to the time of death.

The symptoms of the virus include severe watery diarrhoea, abdominal pain and cramping, nausea and vomiting may follow shortly, with the diarrhoea persisting for a week.

In fatal cases, Amoth says many patients develop severe bleeding manifestations often with fresh blood seen in vomitus and faeces which is accompanied by bleeding from the nose, gums, and vagina.

The largely infectious virus spreads through body fluids in broken skin or mucous membranes and with surfaces such as bedding or clothing contaminated by an infected person. People remain infectious as long as their blood contains the virus.

It is worth noting that the current outbreaks of Marburg virus is quite unprecedented.

The outbreak in Equatorial Guinea is the fourth largest in history and the outbreak in Tanzania the sixth largest.

There has never been two concurrent outbreaks either.
