With perfectly obvious delusions. I’m sure he has a professional diagnosis and refuses to believe it so he’s unmedicated. I’m not making fun of him for that- it would be great if he got true help.

The delusions can be treated but most mentally ill won’t stick to treatment plans. They don’t like not feeling their normal self and cant handle therapy sessions that addresses their true behaviors. They try to snow the therapist but they are truly transparent. They will deny deny deny that anything is their wrongdoing and claim everything was a misunderstanding Sounds awful. Good luck!
