Every country with nukes knows its all in or nothing. If you launch just a one or two on key cities there will be as expected an all out response and you would be remiss to not launch all you have at one time in order to increase odds off a full destructive outcome in your favor. MAD pretty much covers this idea.

It will probably be an EMP if it's a real attack. It will come with the suddenness of a broken shoelace.

Do you think the Japanese were not well aware that they were already fighting Americans?
