I conduct active shooter drills for school districts and businesses.
During said drills, I will typically have one or two "bad guy" role players walking around, yelling threats and generally acting irrational, all while holding either pistol converted to fire only marking cartridges and/or blanks, or a solid blue rubber Glock 17 non-firing replica.
Every single time, no one, I mean NO ONE, sees the pistol in the suspects hand.
They either don't look any further than the suspects face, failing to conduct a whole body scan (which means checking the hands, waistline, torso, etc). They totally tune out the obvious weapon.
That being said, I can see no one noticing a long gun if the perp had made any effort at all to conceal it.
People also live in denial. They will see a threat and dismiss it because they don't want to believe its real, that there are bad people in the world that will hurt or kill them. They also don't want to be accused of causing any hysteria.

You can't just look at people, you have to examine them, constantly scanning the crowd for out of place behavior, checking all the areas I stated above. Don't ignore danger signs, even if you can't articulate it right then and there, your subconscious has picked up and danger signs and is warning you to evacuate the area.
Ignoring that little voice in the back of your head could very well lead to your demise.
