All of the people being monitored have been to mainland China within the past two weeks or were passengers on a cruise ship with a confirmed coronavirus case. No one is being quarantined. The MDHHS says that is because they were not deemed at high risk during the screening process. From the link |

People need to chill out about this I feel like you don't realize the extent of our government's negligence on this issue. The state dept and DHS transported sick ppl and well on the same plane, against the recommendation of the CDC. Furthermore, the President made it clear that we were not to transfer ANY of the sick ppl to American soil. This isn't okay. There were plenty of good reasons to leave the sick ppl there. Heads should roll over this one |

Not getting incubation period right could defeat purpose of quarantining by Rossi A. Hassad PhD, MPH February 19, 2020 As the number of people in quarantine for COVID-19 increases, so does the likelihood of encountering outliers, to the extent that an incubation period beyond 14 days can be considered biologically plausible, and from a statistical and probability perspective. The incubation period is a critical factor in controlling the spread of disease. Another key factor is the latent period, or the time between the occurrence of infection and becoming infectious. This can be shorter or longer than the incubation period, implying that an asymptomatic person may be able to transmit the virus. Both of these factors are crucial to effective quarantine management, but COVID-19 is a new coronavirus, and much remains unknown about its transmission parameters and dynamics. While the World Health Organization (WHO) estimates the incubation period of COVID-19 could be up to 14 days, this upper limit was actually observed for a small proportion of cases of SARS. In the context of an accelerating COVID-19 epidemic and growing uncertainty, a higher upper limit (possibly 21 days) for the incubation period seems reasonable and warranted in the interest of adequately protecting the public. Quarantine measures separate and restrict the movement of people who were exposed to a contagious disease, so as to provide appropriate care if they become ill, and to protect the public from infection. Discharging someone from quarantine before the end of the actual incubation period of the disease can prove disastrous, if that person subsequently develops the disease while not contained. We must get the upper limit of the incubation period right, or we could defeat the purpose of quarantining. One study from China reported an upper limit of 24 days for the COVID-19 incubation period, but the WHO commented that this "can reflect a double exposure" or "outliers," and that the "outlying observation" of 24 days "needs to be taken seriously" but in the "context of all of the other studies." So, the 14-day incubation period remains in effect. According to the CDC, the "symptoms of COVID-19 may appear in as few as 2 days or as long as 14 days after exposure." However, even if we consider the reported single-study upper limit of 24 days to be an outlier observation, and hence unlikely to be encountered in the U.S., the odds may soon swing, with the repatriation of about 400 U.S. citizens from aboard the Diamond Princess cruise ship. These U.S. citizens were potentially exposed to COVID-19, as noted by the CDC. The time that these U.S. citizens spent in quarantine on the cruise ship will not count toward their U.S. quarantine time, as there was active transmission of COVID-19 aboard that cruise ship, and hence no certainty as to when exposure could have occurred. Recent news that an 83-year-old American woman recently tested positive for COVID-19 in Malaysia after disembarking from the cruise ship Westerdam in Cambodia, also reinforces the need to seriously rethink the adequacy of the current 14-day quarantine period, as well as monitoring and screening procedures and mechanisms. The woman was aboard the cruise ship for more than 14 days, was reportedly asymptomatic at the time of disembarking, and hence was not tested for COVID-19 in Cambodia when she traveled by air to Malaysia. Of note, her 85-year-old husband with whom she was traveling has since twice tested negative for COVID-19. Around 200 passengers from the Westerdam have returned to their home countries via Malaysia and Thailand, and none manifested symptoms of COVID-19 before departing Cambodia. Nonetheless, there are heightened concerns about further spread of the disease internationally, but this can be tempered by the report that the woman's husband is not infected, strongly suggesting that she was asymptomatic aboard the cruise ship, and not infectious enough to transmit the virus. The WHO and CDC continue to take a careful and evidence-based approach in responding to the COVID-19 epidemic, which is commendable. Currently, the risk of exposure to COVID-19 for the general public in the U.S. is low. But this is likely not the end of the debate about quarantining, and the delicate balance between "civil liberties" and "the greater good." Rossi A.
