During a five-hour operation to remove a bullet that had pierced Oh's intestines, Lee encountered a complication he's never seen in his 20-year career as a surgeon: parasites. There is another scene in the documentary where all the children are standing in formation outside. They are all given this identical package of what appeared to be very rudimentary toys. Coloring books..maybe a small ball. Little stuffed animal.

It was all identical. The way it was packed. The way they were dressed. The way they were standing. The looks on their faces. There was no joy in receiving their 'present' at all..because it was the same everyone else got.
