Bruce Lee's 20 year old son got shot and killed on the movie set
with a loaded stunt gun.

They don't even do that anymore. For about two decades now they've been swapping guns for shots and the blank guns aren't capabale of passing anything through the barrel. The plug is solid steel with a tiny hole in the center to allow smoke through. They don't even use paper wadded cartriges anymore. Blowback is caused by the force against the plug and the flame effect is done with small remote charges in the barrel. Any shots that require loading or unloading are done with fully plugged barrels and any stunt shots are done with rubber guns.

The only thing I could think of in this case if they were following the rules is somthing got into the barrel of the gun from the front, either accidentally or intentionally or maybe a charge came loose and shot out of the barrel when it was detonated. It's possible they weren't following the rules of course.

The story behind Brandon Lee's death has changed several times over the decades by the way. That version is just the newest one.

Hollowed-out cartridges are often used to film close-ups of a gun being loaded; the “dummy” cartridges are then supposed to be removed and replaced with blanks before being fired. The police investigation into Lee’s death concluded that a tip of one of the cartridge’s bullets broke off from the cartridge and lodged in the gun, then fired at Lee along with the blank.

We're told Alec was filming the scene when someone pulled the trigger. It's unclear if the person who loaded the gun mistakenly placed bullets inside, or if something was lodged in the barrel that hit the director as well as the director of photography.

Movie prop guns are designed where you can't load live ammunition in them. Very odd!
