Aerosol and surface contamination of SARS-CoV-2 observed in quarantine and isolation care


Abstract

The novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) originated in Wuhan, China in late 2019, and its resulting coronavirus disease, COVID-19, was declared a pandemic by the World Health Organization on March 11, 2020. The rapid global spread of COVID-19 represents perhaps the most significant public health emergency in a century. As the pandemic progressed, a continued paucity of evidence on routes of SARS-CoV-2 transmission has resulted in shifting infection prevention and control guidelines between classically-defined airborne and droplet precautions. During the initial isolation of 13 individuals with COVID-19 at the University of Nebraska Medical Center, we collected air and surface samples to examine viral shedding from isolated individuals. We detected viral contamination among all samples, supporting the use of airborne isolation precautions when caring for COVID-19 patients.

Air samples in the rooms and in the hallway spaces (Fig. 1B, and Tables S1 and S2) provide information about airborne viral shedding in these facilities. We found 63.2% of in-room air samples to be positive by RT-PCR (mean concentration 2.42 copies/L of air). In the NBU, for the first two sampling events performed on Day 10, the sampler was placed on the window ledge away from the patient (NBU Room A occupied by Patient 1), and was positive for viral RNA (Table S1; 2.42 copies/L of air). During the sampling event on Day 18 in NBU Room B occupied by Patient 3, one sampler was placed near the patient and one was placed near the door greater than 2 m from the patient’s bed while the patient was receiving oxygen (1 L) via nasal cannula. Both samples were positive by PCR, with the one closest to the patient indicating a higher airborne concentration of RNA (4.07 as compared to 2.48 copies/L of air). Samples taken outside the rooms in the hallways were 58.3% positive (Fig. 1B and Table S2), with a mean concentration of 2.51 copies/L of air. Both personal air samplers from sampling personnel in the NQU showed positive PCR results after 122 min of sampling activity (Table S2), and both air samplers from NBU sampling indicated the presence of viral RNA after only 20 min of sampling activity (Table S2). The highest airborne concentrations were recorded by personal samplers in NBU while a patient was receiving oxygen through a nasal cannula (19.17 and 48.22 copies/L). Neither individuals in the NQU or patients in the NBU were observed to cough while sampling personnel were in the room wearing samplers during these events.

Finally, personal air samplers worn by sampling personnel were all positive for SARS-CoV-2, despite the absence of cough by most patients while sampling personnel were present. Recent literature investigating human-expired aerosol suggests that a large fraction is less than 10 µm in diameter across all types of activity (e.g. breathing, talking, and coughing15) and that upper respiratory illness increases production of aerosol particles (less than 10 µm)16. A recent study of SARS-CoV-2 stability indicates that infectious aerosol may persist for several hours and on surfaces for as long as 2 days"

Disruptive, expensive, distracting. And, the fact you may have just killed grandma, who lives with you, might also be a distraction.

Schools should be closed. Period. Parents need to figure it out. If it means one parent stays home and you have to give up the second car, the new Iphone, cable tv, etc, too fucking bad. One income families were the norm for decades, and it worked. Then Madison Ave convinced us we need to keep up with the Joneses and our emotional worth is tied directly to what we own. The country is in for a very rude awakening, and people are going to have to give up some things in order to make it work. That is the reality. And everyone better get used to the idea.

COMMENTARY: My views on cloth face coverings for the public for preventing COVID-19

Science, when done well, can be messy, imperfect, and slower than we wish. And it's ever-evolving. Unfortunately, in the time of a pandemic, we wish this weren't the case, as we all want and need immediate answers.

Public health policy—including COVID-19 response—should always be informed by the best data available and should evolve with scientific knowledge. But it should not be based on popular opinion or even well-meaning movements within the scientific community. Good science should precede policy, not vice versa.

From the very first cases in China, CIDRAP has been compiling and disseminating science-based information and straight talk on COVID-19. In fact, our dedicated team has been providing science-based, non-partisan coverage of infectious diseases for nearly 20 years. Our only agenda is to inform the public about what the scientific community knows and does not yet know about COVID-19. We take this same approach with cloth face coverings.

At the outset, I want to make several points crystal clear:

I support the wearing of cloth face coverings (masks) by the general public.
Stop citing CIDRAP and me as grounds to not wear masks, whether mandated or not.
Don't, however, use the wearing of cloth face coverings as an excuse to decrease other crucial, likely more effective, protective steps, like physical distancing
Also, don't use poorly conducted studies to support a contention that wearing cloth face coverings will drive the pandemic into the ground. But even if they reduce infection risk somewhat, wearing them can be important.

Georgia second grader tests positive for coronavirus after first day of school, forcing class to quarantine


Two suburban Atlanta school districts that began in-person classes Monday with mask-optional policies face more questions about COVID-19 safety protocols after on-campus pictures showed students packed shoulder-to-shoulder. The country is in for a very rude awakening, and people are going to have to give up some things in order to make it work. That is the reality. And everyone better get used to the idea.

That's why it's not a good idea to send your kids back if you don't have to because they are just going to keep shutting down your kids education every time someone tests positive for a cold virus. There is no way in hell schools will stay open with the up coming cold and flu season under these ridiculous rules. The covid test doesn't distinguish between the covid virus and the regular flu or cold virus because they all belong to the same corona virus family. They are going to be constantly quarantining kids and their families for having a cold virus. In fact the test will come back positive if you have had a cold or the flu in the last ten years that's how faulty the covid test is. Schools can't operate properly under these ridiculous and draconian rules. Better off keeping them home since that's where they will be spending most of the school year anyways because they will constantly be under quarantine if sent to school and subjected to these bogus tests.
